Refer a Patient to One Stop Psychiatry
We take referrals from doctors, nurse practitioners, therapists, schools, hospitals and community programs. We see kids from age 6, teens and adults, at our Fort Lauderdale office or by video anywhere in Florida. Several of our providers and staff speak Spanish.
What you can refer for
- Psychiatric evaluations and diagnosis
- Medication management for depression, anxiety, ADHD, bipolar disorder, schizophrenia, OCD, PTSD and insomnia
- ADHD assessments
- Addiction treatment, including Suboxone, Sublocade, Brixadi and Vivitrol
- Long-acting injections given in our office: Invega, Abilify Maintena, Uzedy, Haldol Decanoate, Brixadi, Sublocade and Vivitrol
- In-home visits (private pay, not covered by insurance)
Same-day visits are often possible, Monday through Saturday, 9 to 5.
How to send a referral
Fill out the referral form on this page, or fax it to (754) 270-6321. Please don’t email patient information.
It helps if you include the patient’s name, date of birth and phone number, their insurance, the reason for the referral, current medications and diagnoses, any recent notes or labs, and your name, practice and a callback number.
If it’s urgent, call us at (754) 270-6322 during office hours.
What happens next
Our staff will reach out to the patient, usually the same business day. We check their insurance or go over self-pay options (we take most plans and have sliding-scale rates), send the intake packet, and confirm the time and whether the visit is in the office or by video. With the patient’s permission, we can send you an update after the first visit.
If labs are needed, patients get an order they can take to Quest, Labcorp or any lab their insurance accepts.
We aren’t an emergency service. For a patient in crisis, call or text 988. If someone is in immediate danger, call 911.
