Bipolar Disorder?

Bipolar disorder is chronic mood disorder characterized by significant mood swings that include emotional highs (mania or hypomania) and lows (depression) the condition, and most people with bipolar disorder also have depressive episodes. These mood swings can affect sleep, energy levels, behavior, judgment, and the ability to think clearly. 

The condition is manageable with medications, talk therapy, lifestyle changes and other treatments.

Types of Bipolar Disorders

Bipolar I Disorder

People with bipolar I disorder have gone through at least one episode of mania. While most individuals with bipolar I will experience both manic and depressive episodes, you don’t necessarily need to have a depressive episode to receive this diagnosis. Depressive episodes last a minimum of two weeks. For a diagnosis of bipolar I, your manic episodes should last at least seven days or be intense enough to require hospitalization. People with bipolar I may also go through mixed states, where they experience symptoms of both mania and depression. 

Bipolar II Disorder 

People diagnosed with bipolar II experience both depressive and hypomanic episodes, but they never have the full manic episodes that define bipolar I disorder.
While hypomania is usually less intense than mania, bipolar II can often be more difficult to manage because chronic depression is more common in this type. 
The hypomanic episodes and depressive episodes may last for days to weeks at a time.

Cyclothymic Disorder

People who have cyclothymic disorder often experience a rollercoaster of emotions. They can swing between hypomania and mild depression for at least two years. While there might be short-lived moments of feeling normal (euthymia), those typically don’t last longer than eight weeks. The symptoms are less severe compared to manic, hypomanic, or depressive episodes associated with bipolar I or bipolar II disorders; however, there are significant fluctuations in mood.

How to Diagnose Bipolar Disorder?

When it comes to diagnosing bipolar disorder, it’s all about a detailed and comprehensive assessment since its symptoms can often be mistaken for those of other mental health conditions. The process starts with a thorough psychiatric evaluation, where a mental health professional will discuss your symptoms, thoughts, feelings, and behaviors with you. Sometimes, this evaluation includes a physical exam to rule out any medical issues that could be impacting your mood swings. It’s really important to provide a full medical history and be candid about any symptoms you’re experiencing to help ensure you receive an accurate diagnosis.

To receive a diagnosis of bipolar disorder, a person must have at least one episode of depression or a manic/hypomanic episode. According to the Diagnostic and Statistical Manual of Mental Disorders (DSM-V), a depressive episode is characterized by several key symptoms, including:

  • A change in appetite, causing weight loss or gain.
  • Irritability.
  • Increased need for sleep or less.
  • Insomnia or excessive sleep.
  • Difficulty concentrating and making decisions
  • Suicidal thoughts

For a depressive episode to be recognized, the individual should experience at least four of these symptoms for a minimum of two weeks. On the other hand, a manic episode must last at least one week and include three or more of the following criteria:

  • Lack of motivation.
  • Feelings of hopelessness or worthlessness.
  • Loss of enjoyment of things that were once pleasurable for you.
  • Increased energy or activity
  • Rapid speech or racing thoughts
